New Account Forms

Thank you for choosing SoCal Rentals. To set up your new account and release equipment to you, we need the following:

1. Rental Agreement

  • Enter the company or individual name on the first page; this must match the name on the Certificate of Insurance (COI) 

 

2. New Account & Credit Application

  • All clients must complete sections I & II
  • If you are seeking credit terms, complete all sections and pages
 

3. Credit Card Authorization

  • A valid credit card for payment is required for all clients
  • If credit terms are granted, the credit card will remain on file for any past due invoices
 

4. Certificate of Insurance (COI)

  Certificate of Insurance (COI) Requirements:

  • Insured’s name must match the name used when your account was opened
  • General Liability coverage must be a minimum of $1,000,000 per occurrence and $2,000,000 aggregate
  • Rental coverage must show coverage of “Rental/Leased Equipment” with coverage written on a “replacement cost basis” and the limit be for the full replacement value of equipment rented, $1,000,000 preferred 
  • SoCal Rentals must be listed as Additional Insured for the General Liability policy and Loss Payee in respect to the rented/leased equipment
  • Certificate Holder information:
